My 63 yr. old Daddy is having Heart Pains and I noticed he had a nosebleed can anyone tell me what's going on?

My Dad is 63 years old and he's an alcoholic and a smoker and for about a week now he's been saying his heart hurts and every time he moves his left arm his heart hurts and he said last night that the pain moved back into his shoulder and I noticed he had previously had a nosebleed I've told him he needs to go to the doctor but when he was at the emergency room with his mom who is old and feeble he decided not to please help I'm curious to know whats going on he's a stubborn old man and I Love Him with all my heart so let me know something so i can give him an Idea of what it might be. Thanks. Oh and also Its Monday he said on Saturday his heart felt like his heart was going to beat out of his chest.
